Azoospermia is a typical male infertility problem where there is an insignificant amount of measurable sperm in the semen. Azoospermia affects about 1% of the male population and may be the cause in about 20% cases of male infertility. The person is unable to produce mature sperms and hence, unable to fertilize the matured egg. Elevation in the body temperature is one of the major causes of this problem. An obstruction can occur due to prior infection, surgery, prostatic cysts, injury, etc. However, the sperms can still be recovered from the testes using microsurgical or endoscopic procedures. TESA is a simple microsurgical procedure used for azoospermia treatment. It involves direct removal of the sperm from the testicles using local anesthesia. Patients usually return to work the following morning after the procedure. It is carried out as an outpatient procedure under intravenous sedation or local anesthesia or a combination of both. The procedure does not take long. Some patients might have ‘islands’ of sperm producing tissue within the testicle. The TESA technique is more likely to pick up such ‘islands’ of tissue as compared to open testicular biopsy. If motile sperm is recovered the sample can be frozen under cryopreservation for several treatment cycles. This saves patients the need for a repeat procedure.
